To protect patient privacy and comply with HIPAA regulations, we require the following for all records requests:
- 1. The patient must contact us directly via email at info@rentoncounselingcenter.com.
- 2. Our official Release of Records form must be completed. We do not accept auto-pen or electronically signed forms.
- A clear copy of the patient’s driver’s license must be included to verify signatures.
