In Person Appointment information:
The office is located in a shopping village in a five-story building called Evergreen Building on Grady & Sunset in the same parking lot as Uwajimaya. Transit center across the street and easy access to major highways 167 and 405. If you are traveling south on 405 you can use exit #3 and go through the light until you reach the building. There is plenty of parking and disabled access on one side of the building.
The office is on fifth flour with a waiting lobby. There is no receptionist, patients must complete their intake paperwork prior to the meeting via Therapy Appointment links. Some providers have a notification light switch. See directory in the room.
For after-hours the building might be locked, calling 425-444-5902 or sending an email or waiting for your provider
to let you in at your appointment time.
